The Hospice LPN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on care, on-going clinical management and education regarding hospice careAs a Hospice LPN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

As a Hospice LPN, you must be caring, patient and flexible teacher with excellent communication and time management abilitiesYou must possess independent problem solving abilities, excellent clinical assessment skills and be computer literateYou must also be in good physical shape and have the ability to lift/transfer patients to and from wheelchairs, if necessary.
